New patient and returning hygiene appointments can now be requested online! Please complete the form below, and we will respond within 1 business day to confirm a convenient time for your child’s appointment. We will also aim to accommodate your scheduling preferences, but will offer alternatives if necessary.

If you prefer to speak with a staff member to schedule your appointment, please call us Monday-Thursday, 8:00am – 5:00pm and
Friday, 7:00am – 2:00pm.
